Community Based Supports (SP)
Individual Supports (CCP)

Community-Based Supports/Individual supports may include but are not limited to, assistance with community-based activities. In addition to training and supervision of individuals as they learn and perform the various tasks included in standard self-care, social skills, finances, medical coordination, and activities of daily living.

Supported Employment
(Individual/Group)

Activities needed to help a participant obtain and maintain an individual job in competitive or customized employment, or self-employment, in an integrated work setting in the general workforce.

*Individual Supported Employment:

1-1 support.

*Group Supported Employment:

2-8 individuals

Support Brokerage

Service to assist individuals and families with the self-direct employee process: application, recruiting, managing workers,providing ongoing information and updates. Additional services: assisting individuals with identifying immediate and long-term needs, developing options to meet those needs and accessing identified supports and services in their communities.

Interpreter Services

Service delivered to a participant face-to-face to support them in integrating more fully with community-based activities or employment. Interpreter services may be delivered in a participant’s home or in a community setting.

Transportation

Door-to-door services currently in the Cumberland and Atlantic counties. We bill through ACUMEN so those who are on either the Supports and Community Care Program Waivers can get to where they need to go. Providing access to transportation allows our clients the freedom and empowerment they deserve.
